Job Description:
A company is looking for a Provider Chart Reviewer.
Key Responsibilities
Review patient medical records to ensure accurate and complete
clinical documentation
Evaluate issues on the issues list and signal providers for
clarification as needed
Identify clinical opportunities and potential diagnoses for
provider review using various tools
Qualifications
Actively-licensed Nurse Practitioner (NP) or Physician Assistant
(PA)
3+ years of clinical practice experience with medical record
review/navigation
Experience with HCC coding practices or Medicare Risk Adjustment
preferred
Ability to work 40 hours per week
Prefer at least 2 years of experience in Primary Care in the last 5
years
