Job Description:
A company is looking for a Clinical Care Specialist to manage
chronic care for patients remotely.
Key Responsibilities
Manage a patient census and coordinate Chronic Care Management
(CCM) services for chronically ill patients
Create individualized Care Plans and monitor labs, vitals,
medications, and testing
Conduct care coordination and provide education on chronic
condition management to patients and their families
Required Qualifications
Registered Nurse (RN) licensure in applicable state
Five years of nursing experience
Experience with chronically ill patients preferred
Telephonic case management or care coordination experience
preferred
Self-motivated and able to work independently
