At Third Street, we're dedicated to delivering high-quality healthcare to our community, and we're looking for an enthusiastic Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team!
**What You'll Do: **
- Provide comprehensive, patient-centered care to individuals and families across the lifespan.- Perform assessments, diagnose conditions, and develop personalized treatment plans.- Collaborate with our multidisciplinary team to ensure integrated care for all patients.- Educate patients and families on health maintenance, disease prevention, and healthy lifestyle choices.- Participate in community outreach initiatives and health education programs.- Maintain accurate and timely patient records in compliance with healthcare regulations.
**Who You Are: **
- APRN or PA State of Ohio License with a focus in Family Medicine.- A strong communicator who builds rapport easily with patients and their families.- Committed to health equity and culturally competent care.- Energetic, adaptable, and eager to work in a collaborative environment.- Experience in a community health setting is a plus but not required.
